Acceptance of Terms

By scheduling and receiving services at Level Up Health & Wellness ("Level Up HBOT"), you agree to be bound by these Terms of Service. These terms constitute a legal agreement between you and Level Up Health & Wellness, located at 123 Wellness Way, Bechtelsville, PA 19505.

Services Provided

Level Up HBOT provides wellness services including but not limited to:

• Hyperbaric Oxygen Therapy (HBOT) sessions
• Bio Harmonic VIBE Bed therapy sessions
• Wellness consultations and assessments
• Package and membership programs

Payment Methods

We accept the following payment methods:

• Visa, Mastercard, American Express, and Discover credit cards
• Debit cards with major network logos
• Cash payments (for single sessions only)
• Check payments (with prior approval)

Pricing and Fees

All prices are clearly displayed and include any applicable taxes. We do not charge hidden fees. Current pricing includes:

• Single HBOT sessions: $120 (60 min) / $160 (90 min)
• Package discounts available for multiple sessions
• Membership plans starting at $250/month
• Special introductory offer: $49 HBOT + FREE 15-min VIBE Bed

Membership Billing

Membership fees are automatically charged monthly to your designated payment method. All memberships require a minimum two-month commitment, after which they become month-to-month. Memberships can be shared with one additional authorized family member.

Appointment Cancellations

• 24-hour notice: Full credit or refund for cancellations made 24+ hours in advance
• Same-day cancellations: 50% charge may apply for cancellations made less than 24 hours in advance
• No-shows: Full session charge applies for missed appointments without notice
• Emergency situations: Evaluated on a case-by-case basis

Membership Cancellations

To cancel your membership:

• Provide 30-day written notice via email to hello@leveluphbot.com
• Complete minimum two-month commitment before cancellation
• Unused sessions from current month are forfeited upon cancellation
• No refunds for partial months

Package Refunds

Package sessions do not expire and are transferable to immediate family members. Refunds for unused package sessions are available within 30 days of purchase, minus a 10% processing fee.

Health Disclosure

You must provide complete and accurate health information, including:

• Current medications and supplements
• Medical conditions and recent surgeries
• Pregnancy status (if applicable)
• Any changes to your health status between sessions

Contraindications

HBOT may not be suitable for individuals with certain conditions. We will screen for contraindications including:

• Untreated pneumothorax (collapsed lung)
• Certain types of lung disease
• Recent ear surgery or injury
• Severe claustrophobia
• Pregnancy (first trimester)

Information Collection

We collect and maintain the following information:

• Personal contact information (name, phone, email, address)
• Health information relevant to HBOT safety
• Payment and billing information
• Session attendance and treatment records

Information Use

Your information is used solely for:

• Providing safe and effective wellness services
• Scheduling and billing purposes
• Emergency contact if needed during treatment
• Compliance with applicable health and safety regulations

Information Protection

We maintain strict confidentiality of your personal and health information. Information is never shared with third parties except as required by law or with your explicit consent. All records are stored securely and access is limited to authorized personnel only.

Assumption of Risk

By participating in HBOT and VIBE Bed therapy, you acknowledge that you understand the potential risks and benefits. While these therapies are generally safe when properly administered, you assume responsibility for any risks associated with treatment.

Limitation of Liability

Level Up Health & Wellness, its owners, employees, and contractors shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services. Our total liability shall not exceed the amount paid for services.

Insurance

Level Up HBOT maintains appropriate business insurance. However, our services are typically not covered by health insurance as they are provided for wellness purposes. Payment is the responsibility of the client.
